Understanding my statement

Payment Options, once elected, will remain the same unless you advise us you would like it changed.

Please note that Payment Options can only be changed at the policy initiation or upon renewal.

The initial Direct Debit plan payment includes a $25.00 service fee.

The Payment Schedule is reflective of the Payment Option chosen or in effect at your last Policy renewal.

The Installment plan is calculated based on 4 equal payments, plus a $35.00 service fee per payment.

Transactions and payments received after this date do not appear on this Statement.

This entry reflects prior Balances billed and not yet paid.

This entry reflects prior Balances currently due.

This entry summarizes changes to your Policy since your last Statement.

This is the due date for payment of your current/new Policy transactions.

This entry reflects Balances that are not yet due, based on the Payment Option noted above.

This entry reflects Balances due for transactions during this period.

This entry reflects the total amount due for prior unpaid Balances and for current transactions.
